Laser hair removal membership agreement

A monthly plan for laser hair removal on the areas recorded in Part B, at Majspa Medspa & Aesthetics, 1401 Johnson Ferry Rd, Suite 100A, Marietta, GA 30062. Treatment is performed on an Alma Soprano Titanium under the medical direction of Dr. Nokuthula Msimanga, MD.

The membership is a payment plan for a course of treatment. It is not a package of prepaid sessions.

Your initial term is twelve months from your first billing date. Your monthly fee is fixed for the whole initial term.

At the end of month twelve your membership automatically converts to the maintenance rate for your tier, shown in Part B. Maintenance then continues month to month until you cancel. The maintenance rate is held for as long as your plan runs without a break.

At least 30 days before your plan converts, we will email you stating your maintenance rate in dollars, the date of your first maintenance charge, and that giving 30 days’ notice to info@majspa.io ends your membership with nothing further to pay. If you cancel after that email and before your plan converts, your plan does not convert and no settlement applies, even if fewer than 30 days remain before the conversion date.

You authorize Majspa Medspa & Aesthetics to charge the card recorded in Part B each month: the monthly fee during the initial term, and then, from month 13, the maintenance rate recorded in Part B unless you cancel before your plan converts. Charges continue until your membership ends under clause 8. Fees are for the plan and are due whether or not you have a session that month.

You are charged on the same day each month as your first billing date. If a charge fails we will tell you and retry after 3 days and again after 7 days. If the account is still unpaid 14 days after the due date, new bookings are paused until it is brought up to date.

Stopping or cancelling the card on file is not a cancellation of your membership. The only way to cancel is the written notice described in clause 8.

You can update your card at any time at the front desk or by contacting us. The signed copy of this agreement you receive under clause 16 is your record of this authorization.

Your membership covers treatment of your included areas at the interval your provider sets, usually every four to eight weeks, because hair only responds to laser during its active growth phase. Most areas need six to eight sessions to reach meaningful reduction. Book through our online booking system or by calling the studio.

Treatments do not bank or roll over. A month without a visit does not add a session later, and sessions have no cash value. This follows from the clinical schedule: a session is due when your hair is ready for it, not when a month has passed.

Please give at least 24 hours’ notice to change or cancel an appointment. A late cancellation or missed appointment incurs a $25 fee, and the session is rebooked at the next clinically appropriate date.

During the initial term you may swap one included area for another of the same size once, at no charge. Any further change, or a change to a larger area, means moving to the tier that covers it.

Maintenance covers the occasional touch-up sessions most people want after completing a course.

The maintenance rate includes up to three touch-up sessions on your included areas in each twelve months, scheduled when your provider advises.

Three business days to change your mind. You may cancel within three business days of signing for a full refund of anything paid, provided you have not had a treatment.

After that, you may cancel at any time by giving 30 days’ written notice. Email to info@majspa.io counts as written notice, and we will confirm receipt in writing.

Before the end of month twelve, cancelling settles at 50% of the membership months left after your 30-day notice period ends, at your monthly rate. Membership pricing is below the per-visit price on the understanding that the term is completed. The settlement is set so that a member who leaves early still pays less per session than the published per-visit price.

After month twelve, maintenance runs month to month. Give 30 days’ notice and there is nothing further to pay.

No settlement is charged if you cancel because of death, permanent disability, a relocation more than 25 miles from 1401 Johnson Ferry Rd, Marietta, or a medical reason that arises after you join and makes laser treatment unsuitable for you. We may ask for reasonable evidence.

A session postponed on clinical grounds under clause 14, for example because of sun exposure, self-tanner, an infection, a medication or pregnancy, is not a cancellation and does not trigger a settlement.

The maintenance rate is earned by completing the initial term. If your membership ends before twelve months and you rejoin later, you rejoin at the membership rates current at that time and a new twelve-month term starts.

If you have prepaid a series of sessions and move to a membership, we credit the value of the sessions you have not yet received, at the series price per session, against your monthly fees.

While your membership is active, including maintenance, you receive 25% off medical-grade skincare on our retail shelf, including Revision Skincare® and ALMA CBD+.

The discount is for your personal use. It does not combine with other promotions and does not apply to gift cards.

Your monthly fee is fixed for the initial term.

We will give you at least 30 days’ written notice before any change to your maintenance rate or to these terms. If you do not accept a change, you may cancel before it takes effect with no settlement.

Your membership is personal to you and cannot be transferred or gifted, because treatment settings are set for your own skin and hair.

This agreement is not consent to treatment. Before your first session you will have a consultation and sign a separate treatment consent.

Your provider may decline or postpone a session on clinical grounds, for example recent sun exposure or self-tanner, an active infection or breakout in the area, certain medications, or pregnancy. You agree to tell us about any change in your health or medications that could affect laser treatment.

Pausing for pregnancy. If you become pregnant you may pause your membership by telling us in writing, including by email to info@majspa.io. The pause starts from your next billing date, and no membership fees are charged while it lasts. Your membership benefits do not expire: the months left in your initial term, and your right to the maintenance rate once that term is complete, carry over and resume where they left off. Your initial term, and the date your plan converts to maintenance, move back by the length of the pause. Billing resumes when you tell us you are ready to restart and your provider confirms laser treatment is suitable for you again. If you decide not to resume, you may cancel under clause 8, and the settlement in clause 8 applies to the months left in your initial term. We may ask for reasonable evidence of the pregnancy.

Laser hair removal produces permanent hair reduction, not permanent removal. Most clients reach roughly 70–90% reduction after a full course, and some follicles regrow over the following years, which is what maintenance is for. Results vary with hair colour, hair thickness, skin tone, hormones and the area treated. No specific result is guaranteed.

This agreement, together with your treatment consent, is the whole agreement between you and Majspa Medspa & Aesthetics about your membership. It is governed by the laws of the State of Georgia. You will receive a signed copy.
